高龄病人超声引导下经皮经肝胆囊引流术的应用及临床价值

The clinical application and value of ultrasonically guided percutaneous transhepatic gall bladder drainage for elderly and high-risk patients

目的探讨高龄高危状态下超声引导经皮经肝胆囊引流术(percutaneoustranshepaticgallbladderdrainage,PTGD)的技术要点及治疗经验。方法超声引导下对13例高龄高危、合并其它重要器官疾病的急性胆囊炎患者施行PTGD术进行回顾性分析。结果除1例造瘘失败外,其余患者均成功置管、术后症状缓解、治愈出院,其中9例患者于12h内症状明显缓解。结论PTGD可迅速解除高龄高危患者急性胆囊炎引起的危急症状,避免因紧急手术引起的手术并发症,充分发挥了超声微创技术创伤少、方便灵活、准确安全的优势;明显降低死亡率、效果突出、值得推广。

Objective To in vestigate the technical key point and therapeutic experience of ultrasound guided percutaneous transhepatic gall bladder drainage(PTGD) for elderly and high risk patients . Methods 13 cases elderly and high risk patients with acute cholecystitis which received PTGD by ultrasound guided were analyzed retrospectively.Results Except for 1 case fail,12 cases of PTGD was performed succefully.The symptoms were relieved after operation and among 9 cases,the symptoms were obviously reliveved within 12 hours.12 cases were cured to discharge.Conclusion PTGD can promptly relieve emergency symptoms for elderly and high risk patients with acute cholecystitis.So,operative complication caused by emergency operation can be avoided.It has less injury,simple manipulation,safety and lower mortality.
