脂Ⅰ方治疗非酒精性脂肪肝合并高尿酸血症临床观察

Clinical observation on treatment of nonalcoholic fatty liver disease complicating hyperuricemia by Zhifang Ⅰ Decoction

目的 :观察脂Ⅰ方治疗非酒精性脂肪性肝病 (nonalcoholicfattyliverdisease,NAFLD)合并高尿酸血症 (hyperuricemia ,HUA)的临床疗效。方法 :将 4 6例NAFLD合并HUA的患者随机分为两组 ,治疗组 2 5例 ,予以中药脂Ⅰ方 (由白术、茯苓、桑寄生、巴戟天、象贝母、粉萆等组成 ) ,水煎服 ;对照组 2 1例 ,予以口服血脂康胶囊。每 8周为 1个疗程 ,连续观察 2个疗程。结果 :治疗组的总有效率为 80 .0 0 % ,优于对照组的 71.4 3% (P <0 .0 1) ;治疗组患者B超影像改善 ,并在临床症状改善方面优于对照组 (P <0 .0 5 ) ;治疗组患者血清尿酸明显降低 (P <0 .0 1) ,两组差别有显著性 (P <0 .0 1) ;两组治疗对丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、γ 谷氨酰转肽酶、总胆固醇、甘油三酯的复常都有明显效果 ,但组间无显著性差异(P >0 .0 5 )。结论

Objective:To observe the therapeutic effect of Chinese herbal recipe ZhifangⅠDecoction on nonalcoholic fatty liver disease (NAFLD) complicating hyperuricemia (HUA).Methods:Forty-six patients suffering from NAFLD complicating HUA were randomly divided into treatment group (25 cases) with ZhifangⅠand control group (21 cases) with Xuezhikang Capsule.One course of treatment was 8 weeks. The data were processed by SPSS 11.0 statistical package after 2 courses of treatment.Results:The total effective rate of the treatment group was 80.00%, which surpassed the control group (71.43%) (P<0.01); ZhifangⅠcould improve the image of B-mode ultrasonography and was better than Xuezhikang in ameliorating the clinical symptoms (P<0.05); ZhifangⅠcould significantly decrease the serum uric acid (UA) (P<0.01), while Xuezhikang had no obvious therapeutic effect on it (P>0.05); ZhifangⅠwas as good as Xuezhikang in recovering alanine aminotransferase (ALT), aspartate aminotransferase (AST),γ-glutamyltransferase (γ-GT), total cholesterol (TC) and triglyceride (TG) (P>0.05). Conclusion:ZhifangⅠhas good effect in treating NAFLD complicating HUA.
