高血压病血液高凝状态与胰岛素抵抗 被引量:2

Relationship between Hypercoagulability and Insulin Resistance in Patients with Essential Hypertension

摘要 目的 探讨高血压病血液高凝状态与胰岛素抵抗的关系。方法 对 86例原发性高血压病人和 4 6例正常人测定血糖、胰岛素 ,纤维蛋白原、凝血酶原时间 ,血小板聚集、活化度 ,t PA、PAI等指标。将高血压病人分为两组即IR和NIR进行研究。结果 与正常人和NIR组比较 ,IR组纤维蛋白原、PAI增高 ;血小板聚集、活化增强 ,微血栓形成 ;凝血酶原时间缩短、t PA降低 (P <0 .0 5~ 0 .0 1 )。且血小板聚集率与Fbg、PAI、INS0 呈显著正相关 (r=0 .5 2 87,r=0 .5 76 4 ,r=0 .5 1 2 9,P均 <0 .0 5 )、与t PA、G AUC/I AUC呈负相关 (r=- 0 .6 5 31 ,P <0 .0 1、r=- 0 .5 2 81 ,P <0 .0 5 )。结论 高血压病存在血液高凝状态 ,可能与胰岛素抵抗有关。 Objective To study the relationship between hypercoagulability and insulin resistance(IR) in patients with EH. Methods 86 patients with EH were included in the study and 46 healthy subjects were used as control. G,Ins were determined. Fbg,PT, PAgR,PAcR,t PA,PAI were also measured.The patients were classify into IR and NIR groups.Results Compared with control、NIR groups, Fbg、PAgR、PAcR、PAI were significantly increased in IR group ( P <0.05~0.01), PT、t PA were significantly decreased in IR group( P <0.05), while there was a microthrombi formation in IR group. PagR ware positively correlated with Fbg、PAI、INS 0( r=0.5287,r=0.5764,r=0.5129 ,P all<0.05);and was negtively correlated with t PA、G AUC/I AUC( r=-0.6531,P<0.01、r=-0.5281,P < 0.05) Conclusion There were hypercoagulable states in EH It is the possible mechanisms of insulin resistance.Microthrombi formation was a mark under the Bradford variable projection microscopy(BVPM) and there was hypercoagulability in patients with EH.
