肠梗阻的多排螺旋CT诊断研究

MSCT diagnosis of bowel obstruction

目的 探讨肠梗阻在多层螺旋CT(MSCT,Multi-slice spiral CT)中的表现和MSCT诊断肠梗阻的优势。方法 回顾性总结28例临床怀疑为肠梗阻病例的MSCT检查结果,并将其中MSCT确诊为肠梗阻的病例与临床手术结果对照。扫描采用MSCT平扫或平扫后增强扫描,成像方法为薄层横断面、多方位重建成像。结果 有23例MSCT诊断为不同程度肠梗阻,其中18例经手术治疗,CT报告梗阻部位正确的为17例,病因诊断正确的为17例。与手术结果对照,符合率均为94％。临床怀疑肠梗阻而MSCT检查后排除的另5例中有3例发现其他病变,MSCT检查结果与手术完全符合。 结论 MSCT对肠梗阻的程度、部位和梗阻原因的诊断及病变区血供情况的了解有重大意义,可作为肠梗阻的首选检查方法。

Objective To study the multislice computed tomography (MSCT) findings and the diagnostic value of bowel obstruction. Methods Twenty-eight cases suspected of bowel obstruction were underwent MSCT with or without intravenous contrast material. The thin slice axial image and MPR image were also analysed. Results To the cases of bowel obstruction, MSCT could find out the obstructive position and show the blood supply. Comparing with the results of surgery, the accurate of the diagnosis of pathogeny was high (94%). To those negative cases, MSCT could also discover some pathology changes which might cause the clinical signs. Conclusions As MSCT can identify the site, level, and cause of obstruction, and show the blood supply of the bowel, it should be suggested as the first choice in diagnosis of bowel obstruction.
