吸入一氧化氮在心胸外科的应用进展

摘要 一氧化氮 (NO)是体内重要的生物活性因子 ,吸入外源性的NO可选择性地降低肺动脉压、改善动脉氧合、减少肺内分流。本文论述了NO治疗心脏手术围术期肺动脉高压的适应证、吸入浓度、治疗结果及注意事项 ;NO在肺移植围术期的应用进展以及在肺切除术后成人呼吸窘迫综合征的治疗价值。 Nitric oxide(NO) is an important biological mediator and a therapeutic tool.Inhaled NO causes selective dilation of the pulmonary vasculature in well-ventilated lung regions and improves ventilation/perfusion matching without affecting the systemic vasculature.The use of inhaled NO produces significant increases in arterial oxygenation and reduces in pulmonary arterial pressures and decreases pulmonary vascular resistance with no apparent effects on systemic hemodynamics.This review provides an update on recent developments pertinent to the role of NO in cardiac surgery,lung transplantation and adult respiratory distress syndrome after pulmonary resection.
