Surgical Treatment of Acute Obstruction Caused by Left Colonic Carcinoma in the Elderly

目的 :探讨老年人左半结肠癌并急性肠梗阻的处理原则及方法。方法 :回顾分析 5 2例 6 0岁以上老年人左半结肠癌并急性肠梗阻的外科治疗。结果 :一期切除吻合 4 5例 (其中 18例使用吻合器 ) ,一期切除近端造口 2例 ,单纯造口 5例。术后吻合口瘘 2例 ,死亡 1例。结论 :重视围手术期处理 ,根据患者具体情况选择手术方式。在条件允许的情况下 ,一期切除吻合是安全可行的。

Objective: To explore the principle and method of surgical management of acute obstruction caused by left colonic carcinoma in elderly. Methods: Clinical data of 52 patients more than 60 years old with acutely obstructed carcinoma of the left colon were analyzed retrospectively. Results: Single-stage resection and anastomosis were performed in 45 cases (18 performed by anastomat). Anastomotic leakage occurred in 2 cases and 1 patient died postoperatively. Conclusion:Special attention should be paid to perioperative management. The rational operation procedure should be selected depends on the patients general condition and local condition of the carcinoma. If it is an indication, the single-stage resection and anastomesis are safe and reliable.
