眼球萎缩合并眶壁爆裂性骨折的手术治疗

Surgical treatment for cases with ocular global atrophy complicated with blowout fracture of orbital bones

目的 探讨眼球萎缩合并眶壁爆裂性骨折的治疗。方法 在行眼球摘除及义眼台植入术的同时 ,通过增加眶内填充物的方法 ,达到一次手术矫正眼窝凹陷的目的 ,共治疗 19例 ( 19眼 )。结果 全部患者术后眼窝凹陷矫正 ,观察 5月至 3年 (平均 19 2月 )。术后 3眼眼窝轻度凹陷。其中 2眼更换义眼外观改善 ,另 1眼眼窝再填充手术而改善。结论 本法可经 1次手术使合并眶壁爆裂性骨折的眼球萎缩患者外观显著改善 ,临床效果较好。

Objective To investigate the treatmnet for patients with ocular global atrophy complicated with blowout fracture of orbital bones.Methods An operation was performed on 19 cases (19 eyes).The operation consisted enucleation of the globe,hydroxyapatite implantation in the orbit and granular hydroxyapatite tamponade inserted posterior to the implant to correct orbital depression.The period of observation was 5 months to 3 years (mean,1.6 years).Results The depression of the orbit was corrected in all the cases.After the surgery,there were 3 cases with mild orbital depression.Among them,2 cases obtained cosmetic improvement after the change of artificial eye,and tamponade operation was performed again on 1 case,then then the ocular appearance was improved.Conclusion The therapeutic effects of the above surgery are good for cases with global atrophy complicated with blowout fracture of orbital bones.
