α-SMA在臀肌挛缩症中的免疫组化研究

摘要 目的:观察α-平滑肌肌动蛋白(alpha-smooth,muscleactin,α-SMA)在臀肌挛缩症的表达、分布。方法押利用免疫组化染色辅以计算机图像定量分析技术,对26例臀肌挛缩症及12例正常臀肌标本α-平滑肌肌动蛋白的表达进行观察和检测。结果:α-SMA在对照组中未见明显表达。12例臀肌挛缩症组织中α-SMA染色阳性,14例臀肌挛缩组织中染色阴性,病变组与对照组两者差异有非常显著性意义(P<0.001);臀肌挛缩症组织7岁以上组α-SMA表达明显减弱,与7岁以下组(包括7岁)比较差异有显著性意义(t=2.364熏P=0.028)。结论:①肌纤维母细胞是臀肌挛缩症发病关键病理因素。②臀肌挛缩症中肌纤维母细胞的表达强度与病程有关,7岁以上组病程较长表达明显减弱,这给临床治疗提供新思路。 Objective:to observe the expression and distribution of alpha-smooth muscle actin(α-SMA)in gluteal muscle contracture and discuss its clinical significance and relationship.Methods:26spec-imens from gluteal muscle contracture were collected between Oct.1994till Oct.2002.The control speci-mens were collected from normal gluteal muscle and fascia.Expression and localization of alpha-smooth muscle actin in different groups were assessed after incubation of paraffin section with a mouse mono-clonal antibody.The expression level ofα-SMA was defected by computer graph analysis system.Result:there was no significant expression ofα-SMA in control group except for the positive reaction in the smooth muscle cells of the small rascvoscularular walls.12specimens of gluteal muscle contracture were positive forα-SMA actin.14specimens of gluteal muscle contracture were negative forα-SMA actin.α-SMA was expressed in the diseased group,which carried a striking difference from its contrasting group(P<0.001).In the gluteal muscle contracture group,the expression ofα-SMA of those aged over7weakened obviously,whose difference from the those aged less than7was of striking importance(t=2.364,P=0.028).Conclusion:①myofibroblast appears to be one of the most important factors in the pathogenesis of gluteal muscle contracture;②the expression intensity of myofibroblast in gluteal muscle contracture is related to history of disease.In the glutead muscle contracture group,the expression ofα-SMA of those aged over7weakened obviously,which gives fresh thought to practical diagnosis and cure of these dis-eases.
