胆道感染时合理选用头孢类抗生素的实验研究 被引量:1

摘要 目的:测定头孢哌酮等七种常用抗生素在胆汁中的药代动力学参数,以便为临床医生治疗胆道感染时提供合理选择抗菌药物的理论依据。方法:犬为实验动物,分别静脉注射头孢哌酮(先锋必),头孢三嗪(罗氏分),头孢他定(复达欣),头孢唑啉(先锋Ⅴ),头孢拉定(先锋Ⅵ),头孢呋新(西力欣),头孢美唑(先锋美他醇)等抗生素后,用微生物法测定胆汁中的药物浓度,经3P87软件进行数据处理,分别得出各抗生素在胆汁中的峰值浓度(Cmax)、峰值时间(Tpeak)、半衰期(T_(1/2)/β)、清除率(CL)、表观分布容积(Va)。结果:先锋必的 Cmax 最高(2464.5μg/ml),罗氏芬的 Cmax 稍低于先锋必,但 T_(1/2)β最长(3024min),先锋Ⅴ,复达欣,先锋Ⅵ的 Cmax,T_(1/2)β低于前二者,但明显高于西力欣,先锋美他醇。结论:胆道感染时,本组七种抗生素中罗氏芬,先锋必为首选药;先锋Ⅴ,先锋Ⅵ虽不及前二者,它们具有经济,药源广的优点,仍可广泛用于治疗胆道感染的抗生素。 AIM This Pharmacokinetic study was performed in order to rationalize selecting Useful- ness of antibiotic for biliary infection. METHODS It was administered separately iv to dogs with CEFOPERAZONE,CEFTRIAXONE, CEFTAZIDIME,CEFAZOLIN,EFRADINE,CEFUROXlME and CEFMETAZONE.Drug concentra- tions in dog's bile were measured by mi-Crobiological method;the maximums of xonxendtration(Cm-Ax), peak time (Tpeak),half-life time (T_(1/2)β),clearance(CL),apparent volume of Distribution (Vd) of every antibiotic in bile were calculatied With 3p87 software. RESULTS The results indicated that Cmax of CEFOPERAZONE was the highest (2464ug/ml), Cmax of CEFTRIAXONE was lower than CEFOPERAZONE,but T_(1/2)β of CEFTRIAXONE was longest (3024min).The Cmax,T_(1/2)β of CEFAZOLIN,CEFTAZIDIME,CEFRADNE was lower than the former two,but they were Significantly higher than CEFUROXIME and CEFMETAZOLE.CONCLUSION This suggest thatin the infection of biliary tract,the CEFTRIAXONE and CEFOPERAZONE were the first choice,although the effect of CEFAZOLIN,CEFURADE was not better than the former two,they can be used to the treatment of infection of biliary tracts widely due to their being ational product,cheap price and extensive source.
