1242例胃癌术后近期并发症分析和治疗

Postoperative Complication in Treatment for Gastric Cancer and its Prevention (Analysis of 123 Cases)

我科1972年6月～1990年12月收治胃癌1442例,手术治疗1242例,术后并发症123例,其中以胸腔感染、吻合口瘘和腹腔感染为主,并发症发生率9.90％。手术治疗术后并发症死亡25例,以吻合口瘘、胸腔及肺部感染为主因。

1242 patients with gastric cancer were treated at our department during Jun. 1972 to Feb. 1990. Among 1242 cases on whom surgical procedures were performed, 1016 cases were classified in the group of gastrectomy with the operation rate 81.80%. The main surgical procedures included total gastrecto- my (210 cases), proximal gastrectomy (147 cases), and distal gastrectomy (659 cases). Postoperative complications were occurred in 123 cases with the complication rate 9.9 %,including anastomotic fistu- la, thoracic and/or peritoneal infection. Twenty-five cases died postoperatively due to persistent anasto- motic fistula,thoracic or lung sepsis. The author analized the major causes and predisposing factors for the postoperative complications encountered. The key problems and measures to prevent them were also emphazised.
