发热门(急)诊传染性非典型肺炎筛查方法探讨

Exploration of screening methods for infections atypical pneumonia in the outpatient and emergency department of fever

目的 探讨发热门 (急 )诊传染性非典型肺炎 (非典 ,重症急性呼吸综合征 ,SARS)筛查方法。方法 对 2 0 0 3年 4～ 10月本院所有门 (急 )诊发热病人按“非典筛查三流程”(初筛流程、发热鉴诊流程、疑似病例处理流程 )进行综合、严谨、动态的非典筛查。结果 共筛查发热病人 35 76人次 ,筛查出疑似病例 4 2例 ,临床诊断SARS病例 7例 ,无一病例漏诊 ,无发生院内交叉感染。结论 非典筛查三流程方法具不易漏诊 ,容易掌握等特点 ,使呼吸道及非呼吸道发热病人被合理分诊 ,SARS病人得到早发现、早隔离、早治疗 ;在当前未有诊断金标准的情况下 ,具有较高的应用价值。

Objective To explore the screening methods for severe acute respiratory syndrome in the outpatient and emergency department of fever.Methods The synthetic,precise and dynamic screening,“the Three Screening Process for SARS”,was performed in all the fever patients in outpatient and emergency department from April 2003 to October 2003.Results The fever patients were screened 3576 times.42 cases of suspected SARS patients were found;7 cases of fever patients were diagnosed as SARS;no case was missed,and there was not cross infection in hospital.Conclusions The screening method for SARS is with less probability of missed diagnosis and it is easy to operete.The fever patients with respiratory tract diseases and non-respiratory tract disorders must be diagnosed dividualy,thus the SARS patients can be early discovered,isolated and treated.It has a higher applicable value in the diagnosis of SARS when there is no diagnostic standard at present.
