小梁切除术联合羊膜植入术的临床应用研究

A Clinical Appficafive Study of Trabeculectomy with Amniotic Membrane Transplanted

目的探讨羊膜植入应用于小梁切除术的临床疗效。方法治疗组随机选择40例青光眼行小梁切除术联合羊膜植入术。对照组随机选择30例,行单纯小梁切除术,随访6个月。结果手术成功率:羊膜组的累计成功率90%,对照组为60%,两组之间差异有显著性(P<0.01)。滤过泡:随访至6个月末时,羊膜组与对照组的累计存活率分别为87.3%和52.3%,经 U 检验,二组间差异有显著性,术后视力较对照组明显提高。术后并发症:二组术后浅前房发生率差异无明显性。结论羊膜应用于小梁切除术可有效地防止滤过泡的瘢痕组织形成,并能有效长期保留功能性滤泡,并发症较少。

Purpose To probe the clinical curative effect of trabeculectomy with amniotic membrane transplanted. Methods 40 cases with glaucoma after trabeculectomy with amniotic membrane transplanted at random.30 cases with glaucoma sfter simplical trabeculectomy were chosen for control group.The follow-up was 6 months.Results Operation success rate:The cumulative success rate in amniotic membrane group was 90%,while that in control group was 65%,The difference between the two groups was significant(P<0.01).Bleb:in amniotic membrane group was 87.3%,while that in control group was 52.3%.U-test difference between the two groups was significant.Vision after operation has obviously improved compared with control group.Complications after operation:The incidence of shallow anterion chamber had no obvious difference between the two groups.Conclusion The application of amniotic membrance in trabeculectomy can prevent the bled from scar formation and can maintain the functional bleb for a long-time with fewer complications.
