胸大肌肌皮瓣修补重度咽瘘临床分析

A Clinical Analysis of Curative Effect of Pectoral Major Muscle Flap in Repairing Severe Pharyngeal Fistula

目的通过对19例重度咽瘘患者采用胸大肌肌皮瓣修补的临床经验总结以评价其疗效。方法全部病例均在局部炎症消退之后全身麻醉手术,切除瘘口处皮肤及下方疤痕结缔组织,修整咽部黏膜并缝合,取胸大肌肌皮瓣覆盖创面及皮肤缺损。结果 14例术后一期愈合,5例再次咽瘘,其中3例经6～8 wk 换药后愈合,2例失访。结论胸大肌肌皮瓣因其良好的血运,较强的抗感染能力,附带的丰富肌肉组织对重度咽瘘的修补疗效满意。。

Purpose To explore the curative effect of pectoralis major muscle flap in repairing severe pharyngeal fistula.Methods All of the 19 cases got operations after local inflammation were controlled.The skin and the scar aroud the fistula were removed.Pectoral major muscle flap were used to repair the tissue's defect after the healthy mucosa were sutured. Results 14 cases were cured.5 cases had pharyngeal fistula again and were cured after surgical dressing change about 6-8 weeks.Conclusion Pectoral major muscle flap is effective and safe in repairing severe pharyngeal fistula for its good blood supply and anti-inflammation ability.
