利多卡因雾化吸入对减轻气管内插管心血管副反应的研究 被引量:3

摘要 目的 观察雾化吸入利多卡因以消除气管内插管心血管副反应的效果。方法  3 0例ASAⅠ~Ⅱ级择期手术患者 ,随机分为三组 ,每组 10例。全麻诱导采用咪达唑仑 5mg、普鲁泊福 2mg/kg和维库溴铵 0 .15mg/kg静脉注射。Ⅰ组在诱导前雾化吸入生理盐水 10ml(对照组 ) ;Ⅱ组在诱导前雾化吸入 2 %利多卡因 10ml;Ⅲ组在诱导前雾化吸入 2 %利多卡因 10ml,再静脉注射芬太尼 2 μg/kg。 结果 Ⅲ组在插管后SBP ,HR和RPP变化最小 ,与基础值比较无显著差异 (P >0 .0 5 )。Ⅲ组与组Ⅰ ,Ⅱ同期比较均显著性降低 (P <0 .0 5 )。结论 雾化吸入 2 %利多卡因10ml和伍用静脉注射芬太尼 2 Objective To assess the effect of aerosilized lidocaine on cardiovascular response in endotracheal intubation.Methods Thirty patients,ASA classⅠ~Ⅱ,aged 18~60 years,scheduled to elective operations were randomly allocated to receive aerosolized normal saline as placebo(group Ⅰ);aerosolized 2% lidocaine 10ml(group Ⅱ) and aerosolized 2% lidocaine 10ml combined with intravenous fentanyl 2μg/kg before endotracheal intubation(group Ⅲ) respectively.Anesthesia induced by intravenous administration of midazolam, propofol and norcuron.The ECG,SBP,HR and RPP were recorded before aerosolized inhalation and 10 min after endotracheal intubation.Results The parameter values kept statistically stabilization in group Ⅲ compared with group Ⅰ and group Ⅱ.The SBP,RPP values decreased markedly in group Ⅲ following intravenous induction.Conclusion Aerosolized liodocaine combined with low-dose fentanyl can be apply in clinic to stable the hemodynamic status during endotracheal intubation.
