异丙酚复合芬太尼麻醉在超声引导微波治疗肝癌中的应用 被引量:3

摘要 目的 评价异丙酚复合芬太尼麻醉在超声引导下微波凝固法治疗肝癌中的应用效果。方法  6 0例肝癌患者 ,随机分为对照组和观察组。对照组 30例 ,局麻下行微波治疗 ,静脉不使用镇痛药。观察组 30例 ,治疗前先静滴芬太尼 0 .5~ 1μg·kg-1,然后静脉推注异丙酚 1.5~ 2mg·kg-1,微波凝固过程中根据患者反应追加异丙酚 0 .5~ 1mg·kg-1/次。对比观察二组镇痛效果、BP、HR、SpO2 及治疗时间。结果 观察组与对照组比较微波凝固时镇痛效果显著 (P <0 .0 5 ) ;二组患者微波凝固时的BP和HR有显著性差异 (P <0 .0 5 ) ;二组SpO2 和治疗时间无显著性差异 (P >0 .0 5 )。结论 异丙酚复合芬太尼麻醉能够满足超声引导下微波凝固法治疗肝癌的需要 ,明显减轻了患者的痛苦 。 Aim To evaluate the effect of propofol plus fentanyl anaesthesia in the percutaneous microwave coagulation therapy (PMCT) of hepatocellular carcinoma (HCC).Methods 60 HCC patients were randomly assigned to two groups:a control group of 30 patients given local anesthesia and an observation group with 30 patients given vein anesthesia of propofol and fentanyl respectively during PMCT for HCC.The analgesic effect,BP,HR,SpO 2 and the operational duration were observed.Results The analgesic effect was significant in the observation group (P<0.05).The difference in BP and HR between the two groups is significant (P<0.05).There was no significant difference in SpO 2 and the operational duration (P>0.05).Conclusion Propofol plus fentanyl anaesthesia is satisfying in the percutaneous microwave coagulation therapy for hepatocellular carcinoma,can effectively reduce the pain of HCC patients,and keep the BP and HR smoothing during the PMCT.
