微型螺钉内固定治疗桡骨头粉碎性骨折

Internal fixation by using mini-type screws for comminuted fracture or radial head

目的 运用微型螺钉治疗桡骨头粉碎性骨折以改善肘关节功能。方法 15例闭合性桡骨头粉碎性骨折 ,按Morrey改良Mason分型 ,Ⅲ型骨折 13例 ,Ⅳ型骨析 2例 ,合并肱骨内髁骨折 6例 ,尺骨喙突骨折 2例 ,均采用AO微型螺钉进行固定。术后 1～ 3天开始功能锻炼。结果 所有病例关节面均获得满意复位。随访 6～ 2 3个月 ,平均12 5个月 ,根据Mackay和Carn疗效评定 ,优 11例、良 3例、差 1例 ,优良率 93 3%。结论 微型螺钉内固定治疗桡骨头粉碎性骨折是一种有效的方法。

Objective To treat comminuted fracture of radial head by using mini-screw for the improvement of elbow function. Methods Fifteen cases with comminuted fracture of radial head were collected. According to Morrey classification there were 13 of type Ⅲ, 2 of type Ⅳ, 6 associated with medial epichondylar fracture, 2 with of ulnar coracoid fracture. All of them were treated surgically by using AO mini-screw fixation. Rehabilitation was begun 1 to 3 days after operation. Results All cases get satisfactory replacement of the joint surface. The follow-up time was 6-23 months, averaged 12.5 months. Evaluation of Mackay and Carn method was excellent 11 cases, good 3 and bad 1. Conclusion Mini-screw internal fixation is an effective method for management of radial head fracture.
