猫抓病CT表现与病理诊断

CT Manifestations of Cat-scratch Disease

目的 评价CT对猫抓病的诊断价值 ,提高对猫抓病CT诊断水平。资料与方法 分析 12例经临床及病理证实的猫抓病的CT表现并与病理对照。结果 12例CT像上均见病变区肿大淋巴结 ,直径 1～ 6cm ,增强扫描轻度强化 3例 ,环形强化 1例 ,无强化 2例。其病理改变为 :组织细胞增生期 2例 ,肉芽肿形成期 2例 ,脓肿形成期 8例。结论 CT可显示猫抓病所引起的淋巴结炎性肿大 ,结合临床有猫抓、咬伤史 。

Objective To evaluate CT in the diagnosis of cat-scratch disease.Materials and Methods CT findings in 12 cases with clinically- and pathologically-proved cat-scratch disease were retrospectively analyzed and compared with pathologic results.Results On CT scan, lymphadenopathy was found in all 12 cases. The enlarged lymph nodes had a diameter of 1～3 cm. On enhanced scanning, the lymph nodes showed mild enhancement in 3 cases, circular enhancement in one case and no enhancement in 2 cases. Pathologically, the disease was in histiocytosis stage in 2 cases, in granulation stage in 2 cases and in abscess stage in 8 cases. Conclusion CT can detect lymphadenopathy caused by cat-scratch disease. A combination of lymphadenopathy with clinical history of cat scratching is very helpful for the diagnosis of the disease.
