家庭心理干预对首发精神分裂症患者社会功能、家庭环境及复发率的影响

Influence of family mental intervention on social function,family environment and relapse rate in first episode schizophrenics

目的:探讨家庭环境及其家庭心理沟通对首发精神分裂症的干预效果。方法:采用随机抽样方法把驻马店市精神病医院86例首发精神分裂症住院患者随机分为研究组和对照组。研究组从患者精神症状基本缓解开始实施家庭心理干预治疗和药物治疗,对照组仅接受药物治疗。对入组患者使用BPRS、FES-CV和SDSS进行比较。结果:经9个月的家庭心理干预后,研究组患者和家属FES-CV的家庭亲密度、情感表达、成功性、文化性、娱乐性和控制性明显增加,矛盾性明显降低(t=2.00～3.68,P<0.05～0.01)。BPRS总分及自知力因子分与对照组相比明显降低(t=6.52～11.70,P<0.01)。1年后随防,两组患者的治愈率、复发率和病残率均有显著差异(χ2=4.68～5.17,P均<0.05)。结论:家庭心理干预治疗可改善精神分裂症患者的家庭环境,有利于提高疗效,改善社会功能,防止精神分裂症的复发有重要作用。

AIM:To investigate the intervention effect of family environment and family mental communication in first-episode schizophrenics. METHODS:A total of 86 patients with first episode schizophrenia were selected in Zhumadian Municipal Psychiatric Hospital with random sampling method.The patients were randomized into study group and control group.Patients in the study group received the family mental intervention treatment and medicine treatment from the beginning of the basic alleviation,while those in the control group only received the medicine treatment. Comparison between the two groups was carried out with SDSS,BPRS,FES CV. RESULTS:After nine months of family mental intervention,FES CV showed that the intimate degree,emotion expression,success,culture,amusement and controlling nature of patients and family members in the study group were significantly increased,while the contradiction lowered remarkably(t=2.00-3.68,P< 0.05-0.01).The total score and insight factor score of BPRS were obviously lower than those of the control group(t=6.52-11.70,P< 0.01).After one year follow up,the rates of cure,relapse and disability of the patients in the two groups had distinct differences(χ2=4.68-5.17,all P< 0.05). CONCLUSION:The family mental intervention treatment can ameliorate the family environment of patients with schizophrenia,improve curative effect,modify the social function,and prevent schizophrenia from relapsing.
