奥扎格雷钠治疗难治性不稳定型心绞痛的疗效观察及其对血小板活性的影响

EFFECT OF OZAGREL SODIUM INJECTION ON REFRACTORY UNSTABLE ANGINA PECTORIS AND ITS INFLUENCE ON PLATELET ACTIVITY

目的评价奥扎格雷钠治疗难治性不稳定型心绞痛的疗效及其对血小板活性的影响。方法将58例不稳定型心绞痛患者分为二组经常规治疗1周后心绞痛发作次数在此1周内仍≥5次者称之为难治性不稳定型心绞痛(观察组);<5次者判定为治疗有效(对照组)。比较二组患者奥扎格雷钠治疗前血小板活性指标和临床指标的差异及奥扎格雷钠治疗后对观察组血小板活性指标和临床指标的影响。结果观察组的既往糖尿病史和陈旧性心肌梗死史明显高于对照组(P<0.05),二组患者的心电图比较无显著性差异,与对照组比较,观察组的血小板聚集率和血浆TXB2明显增高(P<0.001);观察组应用奥扎格雷钠治疗之后,心绞痛发作频率由常规治疗时的每周发作(10.1±2.0)次减少至(3.0±1.2)次(P<0.01),心电图NST和ΣST较治疗前也有显著改善(P<0.05和P<0.01),与治疗前相比,在应用奥扎格雷钠治疗之后,血小板聚集率和血浆TXB2显著降低(P<0.001)。结论奥扎格雷钠对难治性不稳定型心绞痛有较好的临床疗效并可降低其血小板活性;血小板活性升高在一部分不稳定型心绞痛的发病机制中可能起一定作用。

Objective To evaluate the effect of Ozagrel sodium injection on refractory unstable angina pectoris and its influence on platelet activity. Methods 58 patients with unstable angina pectoris were divided into two groups.Frequency of angina onset was≥5 times and<5 times per week by regular therapy was defined as observation group and control group.Platelet activity and clinic factors of two groups before therapy of Ozagrel sodium were compared.Influence of Ozagrel sodium on Platelet activity and clinic factors of observation group was studied. Results Prior diabetes mellitus and old myocardial infarction in observation group were more than in control group(P<0.05).There was no significant differences between two groups’ electrocardiograms.As compared with control group,levels of platelet aggregation rate and serium TXB 2 were raised in observation group(P<0.001).Frequency of angina onset in observation group were(10.1±2.0) times per week before before therapy of Ozagrel sodium and (3.0±1.2)times per week after therapy of Ozagrel sodium(P<0.01). Electrocardiograms NST and ΣST were improved than before therapy of Ozagrel sodium (P<0.05 and P<0.01). Levels of platelet aggregation rate and serium TXB 2 were significantly decreased after therapy of Ozagrel sodium(P<0.001). Conclusion Ozagrel sodium injection could effectively treat refractory unstable angina pectoris and lower its platelet activity.Raised platelet activity may play a role in the pathogenesis of some unstable angina pectoris.
