佩尔地平静脉滴注治疗老年高血压危象的临床研究

CLINIC RESEARCH ON SENIOR HYPERTENSION CRISIS THROUGH VEIN INSTILLATION OF PERDIPINE

目的观察佩尔地平对老年高血压危象的疗效及安全性。方法老年(≥60岁)高血压危象患者92例,治疗前平均收缩压(236±16)mmHg和/或舒张压(125±11)mmHg,以恒速输液泵持续静滴佩尔地平,剂量为0.5～6μg/(kg·min),根据血压每5min调整一次剂量,每次增加5μg/min直至达到目标血压150～160/100～110mmHg,然后维持此血压水平1～3d。结果用药后5min、10min、30min、1h、2h的平均血压降至231/125,226/123,217/121,207/118,203/107mmHg,24h后达到最大效应,血压平均为(168±11)/(100±9)mmHg,并稳定至用药结束。未发现严重不良反应。结论佩尔地平治疗老年高血压危象起效快,降压作用平稳,安全性高。

Objective observation of the therapeutic effects and securities of Perdipine on senior hypertension crisis. Methods 92 senior(≥60)hypertension sufferers were investigated.The average shrink blood pressure (SBP) and disstole blood pressure (DBP)before treatment were (236±16)mmHg and (125±11)mmHg respectively.Perdipine (Nicardipine) was constantly instilled into sufferer's veins using automatic instillation pump.The adapted doses were 0.5～6μg/(kg·min) and gradually increased every 5 minutes according to the measured blood pressures.Then the measured blood pressure were kept at 150～160/100～110mmHg for 1～3 days. Results After 5min、10min、30min、1h and 2h treatment,the average blood pressure dropped to 231/125mmHg、226/123mmHg、217/121mmHg、207/118mmHg and 203/107mmHg respectively.After 24 hours,the maximum effect was obtained and the average blood pressure was stabilized at (168±11)/(100±9)mmHg until the end of treatment.No obvious side effects were observed. Conclusion Perdipine has significant therapeutic effects for fast and stable blood pressure reduction as well as high securities on senior hypertension crisis.
