重症败血症患者血小板膜糖蛋白的变化及其意义

摘要 目的 探讨重症败血症患者血小板功能状态的变化及其意义。方法 检测26例重症败血症患者的外周血中血小板膜糖蛋白CD62P、CD61以及血浆血栓素B2(TXB2)和6-酮一前列腺素F1a(6-keto-PGF1a)的变化,与对照组比较分析。并把26例重症败血症患者分为器官功能不全组和正常组进行比较分析。结果 重症败血症患者血小板膜糖蛋白CD62P和CD61及FXB2明显升高,与对照组比较有显著性差异(P均<0.01);而6-keto-PGFln。较对照组明显下降(P<0.05)。器官功能不全组血小板膜糖蛋白CD62P和CD61以及TXB2均显著高于器官功能正常组(P均<0.05);而6-keto-PGF1a两组比较无显著性差异。结论 重症败血症患者血小板处于相对的高激活状态,容易发生多器官功能障碍和DIC的危险,以及血小板激活指标可作为判断脏器功能损害的指标之一。 Objective To investigare the clinical significance of platelet membrance glycoprotein in patients with septicemia. Methods 26 patients with septicemia were enrolled. Patients were distributed into two groups according to clinical pattern: organs function failure or normal organs function group. The control group consisted of 25 health people. The expression of platelet membrance glycoprotein CD62P,CD61 were assessed and serum level of TXB2 and 6-keto-PGFla were also assessed. Results Significant increase of Platelet membrance glycoprotein CD62P,CD61 expression and serum level of TXB2 were observed in patients with septicemia when compared with control group (P<0.01). And significant decrease of 6-keto-PGF1a were observed in patients with septicemia when compared with control group (P<0.05). The expression of platelet membrance glycoprotein CD62P,CD61 and serum level of TXB2 in organs function failure group was significantly higher than those in normal organs function group (P<0.05). These are no significant difference in serum level of 6-keto-PGFta(P> 0.05) between two groups. Conclusion platelets were highly activated in patients with septicemia which might contribute to an increased risk for MOF and DIC. The platelet activation might be one of marker of MOF.
