中脑周围非动脉瘤性蛛网膜下腔出血临床分析

The clinical study of perimesencephalic nonaneurysmal subarachnoid hemorrhage

目的 :探讨中脑周围非动脉瘤性蛛网膜下腔出血 (PNSH)的临床特征。方法 :分析 18例PNSH患者床表现、CT特征、治疗及预后。结果 :本组PNSH患者均无意识障碍 ,Hunt-Hess分级Ⅰ～Ⅱ级。均有头痛 ,呕吐 15例 ,脑膜刺激征阳性 14例 ,局灶性神经功能障碍 2例。所有病例均治愈出院。结论 :PNSH患者临床症状轻 ,并发症少 ,预后好。

Objective:To explore the clinical features of perimesencephalic nonaneurysmal subarachnoid hemorrhage (PNSH).Methods:The clinical manifestation,CT,treatment and prognosis of eighteen cases were analyzed.Results:All patients were conscious,whose Hunt-Hess scale were Grade Ⅰ to Ⅱ.In 18 cases headache developed,15 (83.3%) vomiting,14(77.8%)meningeal irritation,and 2(11.1%) focal neurologic deficits.All patients recovered completely.Conclusion:PNSH is a distinct clinical entity with few manifestations,good prognosis and low rate of severe complications.Repeated angiography should be avoided for PNSH.
