实验性家兔心脏和肾脏短暂缺血对急性心肌梗死后QT离散度影响的比较研究

摘要 目的 分析心脏和肾脏短暂缺血预处理对急性心肌梗死 (AMI)后QT间期离散度 (QTd)变化的影响。方法 观察AMI组 (A组 )、心脏缺血预处理 (MIP)组 (B组 )和肾脏缺血处理 (RIP)组 (C组 )新西兰兔在AMI前后的QTd ,并行心肌电生理诱发室性心动过速 (VT) ,对三组的指标进行比较 ,同时设假手术组 (D组 )作对照。结果 四组在手术前QTd比较差异无统计学意义 (P <0 .0 5 ) ;D组手术前后QTd比较差异无统计学意义 (P >0 .0 5 ) ;在AMI后 ,B、C二组QTd明显小于A组 (P <0 .0 5 ) ,而B、C二组QTd比较差异无统计学意义 (P >0 .0 5 )。结论 MIP和RIP均可减少AMI后QTd及室性心律失常的发生 。 Objective To study the influence of myocardial infarction preconditioning(MIP) and renal infarction preconditioning(RIP) on QT dispersion(QTd) after acute myocardial infarction(AMI). Methods QTd of group A(rabbits without IP),group B(rabbits with MIP) and group C(rabbits with RIP) were measured after AMI, and electrophysiologic examination was made to induce ventriuclar tachycardia(VT).The gained indexes of three groups were analyzed and compared with each other. Group D(shame operation)was served as control. Results QTd of each group had no significant changes before operation(P>0.05).QTd of group D had no significant changes before and after operation(P>0.05).After MIP QTd of group B and group C after AMI were significant lower than that of group A(P<0.05).Group B was well-matched with that of group A(P>05). Conclusion QTd and the rate of VT in group B(with MIP) and group C(rabbits with RIP) are significant lower than that of group A(without IP),and the treatments of MIP and RIP have no significant changes.
