恶性局限型胸膜间皮瘤CT诊断

CT diagnosis of local lesion in the malignant pleural mensothelioma.

目的 探讨恶性局限型胸膜间皮瘤CT表现与病理关系以及CT诊断价值。方法 收集 1994年 1月至 2 0 0 3年 12月间经临床或病理证实的 13例恶性局限型胸膜间皮瘤进行综合分析。结果 13例患者中 ,位于左侧胸腔 8例 ,右侧胸腔 5例 ;位于胸腔上半部者 3例 ,位于肺门区 1例 ,其余位于胸腔下半部 ;位于侧、后胸壁者 9例 ,仅 1例位于前胸壁。胸腔积液 9例 ,1例发现对侧肺内转移 ,2例可见钙化 ,颈椎转移 1例。结论 CT能清楚显示肿瘤与胸膜关系 ,以及胸膜病变的部位形态和分布。在CT引导下做胸膜穿剌活检 ,获取足够的标本供组织学检查非常必要。

Objective To analyze the CT findings for local malignant pleural mesothelioma and determine the value of CT in the diagnosis of plrural diseases.Methods The CT scans of 13 patients whit local malignant pleural mesothelioma proved histologically were retrospectively reviewed,and the differential diagnosis of pleural diseases was also discussed. Results There were 8 cases with located the left thorax,the others situated the right thorax.3 cases located in the upside half of the thorax,1 case located hilum of lung,the others situated the second half .Pleural effusion were observed on CT scans in 9 patients,1 case metastatic tumor were seated opposite lung ,2 cases were observed partial calcification,and 1 patients had metastsis which extended cervical vertebra.Conclusion CT could play important role in detecting the location,extent of pleural lesions and follow-up of patients.
