运动试验结合动态血压监测对老年高血压的早期检出

Diagnostic value of exercise test in combination with 24 hours ambulatory blood pressure monitoring for aged patients with hypertension

目的:活动平板运动试验检出运动高血压,并结合24h动态血压监测(ABPM),探讨其早期检出老年高血压的价值。方法:采用修正Bruce方案行症状限制性次极量运动试验,共检出45例老年运动高血压患者,并行ABPM检查。结果:45例老年患者静息血压均在正常范围,运动峰值血压高达(211.98±9.37)mmHg/(101.50±12.86)mmHg,其中ABMP符合高血压诊断标准的有14例;与ABMP正常者相比,前者ABMP各项参数均显著高于后者(P值均<0.001)。结论:平板运动试验结合动态血压监测有助于早期发现老年高血压。

Objective: To investigate the diagnostic value of exercise together with 24 hours ambulatory blood pressure monitoring (ABPM) for aged patients with hypertension. Methods: Aged patients performed symptom restrictive exercise test according to modified Bruce program to detect hypertension induced by exercise and then underwent ABPM. The blood pressure (BP) was normai at rest in these patients. Results: During exercise 45 patients complicated with hypertension induced by exercise, whose peak value of BP reached (211. 98±9. 37) mmHg/ (101. 50±12. 86) mmHg. In which 14 patients were consistent with the diagnosis of hypertension according to ABPM, and all the parameter of ABPM were greater in these 14 patients than those in normai BP patients during ABPM (P<0.001, respectively). Conclusion: Exercise test combined with ABPM may help to find hypertension early in the aged.
