中西医结合治疗创伤性膝关节滑膜炎

Treatment of knee joint acute traumatic synovitis with combined therapy of traditional Chinese medicine and western medicine

目的报道以中西医结合治疗膝关节急性创伤性滑膜炎的疗效。方法损伤2～3d内膝关节肿胀不严重,用屈伸膝关节手法治疗。损伤后3～5d内,局部触诊时皮肤肿胀呈浮髌者,采用穿刺抽吸积血。5～15d积血瘀化块状时,触诊仍有波动感者,用小切口方法彻底清除积血,膝关节加压包扎制动方法。同时配合中药内服或局部外敷。结果97例均获得12个月随访,全部病例膝关节功能及肌力恢复正常,手法治疗15例中优14例,良1例。穿刺治疗46例中优41例,良5例。小切口治疗36例中,优29例,良7例。结论中西医结合治疗操作方法简单,疗效满意,适合基层临床使用。

Objective To report the curative outcomes of the treatment of knee joint acute traumatic synovitis with the combined therapy of traditional Chinese medicine and western medicine. Methods 96 cases of knee joint acute traumatic synovitis were treated with the combined therapy in my hospital after 1997. Those whose injury history was only 2 to 3 days were treated with hand manipulation to flex and stretch the joint if there was no serious swelling. Those whose injury occurred 3 to 5 days before were treated with puncture to draw out hematocele if there was swelling. When the trauma occurred 5 to 15 days before and the hematocele accumulated into ecchymosis inside the joint, a small incision was made to thoroughly clear the ecchymosis and the knee joint was immobilized. At the same time, all the patients were given Chinese herbal medication which was administered orally and/or applied externally. Results All the cases were followed up for 12 months. The function of the knee joint and the strength of the muscles recovered to normal in all the cases. Of the 15 cases treated with had manipulation, 14 were excellent and 1 fine. Of the 46 cases treated with puncture, 41 were excellent and 5 fine. Of the 36 cases treated with incision, 29 were excellent and 7 fine. Conclusion In treatment of knee joint acute traumatic synovitis, the combined therapy of TCM and Western medicine is easy and simple but can result in satisfactory results. It is suitable especially for the grass-root clinics in China.
