磁共振胰胆管成像对胆道梗阻的诊断

Diagnosis of Biliary Obstruction Diseases with the Valueof MR Cholangiopancreatography

目的 :探讨磁共振胰胆管成像 (MRcholangiography ,MRCP)对胆道梗阻的诊断价值。方法 :用重T2 加权完成 37例MRCP检查 ,全部病例均经手术病理证实。经二维 (2D)处理图像 ,然后以最大信号投影重建图像。结果 :全部病例都获得成功。良、恶性胆道梗阻在MRCP图像上具有一定的特征性 ,结合SE/T1 、FSE/T2 及胰胆管成像的原始图像 ,MRCP定位准确性为 1 0 0 % ,定性准确性为 84 %。结论 :MRCP是一种无创伤的检查方法 ,能清楚、直观地显示正常或异常的胰胆管结构 ,结合常规扫描所见 ,定性准确性较高 。

Objective:To evaluate the diagnostic value of MR Chlangiopancreatogaphy in diagnosis of pancreatico-bilitary duct obstructive diseases.Methods:MRCP was performed in 82 cases with 2-D heavy T_2WI fast spin echo sequence,then the resulting slices were projected into a composite image using a standard maxium intensity projection algorithum.All of this cases were verified with operation and histology.Results:MRCP were satisfactorily performed in all cases.Refer to SE/T_1WI SE/T_2WI and primary imaging,the malignant or benign obstructive diseases of pancreaticobiliary duct might be shown characteristic appearances in MRCP.The accuracy of MRCP for evaluating the site and the causes of these diseases were 100% and 84% respectively.Conclusion:MRCP is a noninvasive technique for depicting the normal and abnormal pancreaticobiliary duct.accuracy in the determination of the locations and cases.It can provide much valuable information for the clinical application.
