B型钠尿肽测定对急性呼吸困难病因鉴别的价值

摘要 目的 探讨B型钠尿肽 (BNP)对急性呼吸困难病因的鉴别诊断价值。 方法 免疫荧光法测定 82例急性呼吸困难患者的血BNP水平 ,以BNF =10 0ng/L为界值对其诊断充血性心力衰竭 (CHF)的价值进评价 ,并观察心功能分级与BNP水平的关系。 结果  82例诊断为CHF 5 4例 ,非CHF 2 8例 ,两组的BNF水平有非常显著的统计学意义 (P <0 .0 1)。BNP≥ 10 0ng/L时对CHF的诊断敏感性为92 .4 % ,特异性为 91.3% ,阳性预测值为 90 .7% ,阴性预测值为 92 .1%。心功能 (NYHY)分级越高 ,患者BNF的水平也越高。 结论 BNP在急诊中用于鉴别急性呼吸困难病因有肯定的价值。 Objective To investigate the value of B-type natriuretic peptide (BNP) levels in differentiating congestive heart failure (CHF) from lung disease in patients with dyspnea. Methods Immunofluorescence method was used to mesure BNP levels in 54 patients with CHF, 28 patients with pulmonary dyspnea without CHF. Results (1) Taken≥100 ng/L as cut-off level, the sensitivity of BNP was 92.4% and specificity was 91.3% for diagnosis of CHF with a positive and negative predictable valure of 90.7% and 92.1% respectively. (2) BNP levels in CHF group, dyspneic patients without CHF group were (717.5±394.7)ng/L, (54.7±46.8)ng/L,respectively. The BNP levels were much higher in CHF patients then those dyspneic patients without CHF(P<0.01). (3) BNP level had a significant positive correlation with NYHY class(P<0.01). Conclusion BNP assay had an important clinical role in differentiating CHF from lung disease in patients with dyspnea.
