单侧胫骨远端骨牵引垂直悬吊治疗婴幼儿股骨干骨折

Vertical sling with unilateral distal tibia skeletal traction on femoral fractures of infants

目的 探讨婴幼儿股骨干骨折的有效治疗方法。方法 以 X线平片为参照 ,用细克氏针在患侧胫骨远端作骨牵引单侧垂直悬吊治疗婴幼儿股骨干骨折 15例。牵引 3～ 4周 ,经 X线片证实骨折处有骨痂后 ,去除牵引 ,使用夹板或髋人字石膏保护。结果 随访 1年 ,15例患儿均达骨性愈合 ,愈合时间为 4～ 8周 ,无皮肤过敏和水泡发生 ,无下肢血运受限 ,无终止治疗者。下肢不等长 6例 (在 1.5 cm之内 ) ,无内外成角者 ;3例有 5°左右的旋转 ,但是髋膝关节活动度正常 ,无跛行。结论 单侧胫骨远端骨牵引、垂直悬吊治疗婴幼儿股骨干骨折 。

Objective To explore effective treatment for femoral fractures of infant.Methods 15 patients with femoral fractures were treatded by vertical sling with unilateral distal tibia skeletal traction.Skeletal traction was removed about 3 to 4 weeks,then the lower limb was protected by splint or single leg spica cast until there was radiographic evidence of complete healing with mature callus on both anteriposterial and lateral radiographs.Results All patients was followed up one year ,there were no cases of ununion ,the time of union was 4 to 8 weeks.There were no cases of skin hypersensitivity reaction ,bubble and angulation.All treatment course in the group wasn't interrupted ,6 cases have leg-length discrepancy ( rang 0 to 1.5 centimeter) ,and 3 cases were found about 5° rotation,but all patients had normal function of knee and coxa.Conclusion This is a simply and effective method for femoral fractures of infant ,it has fewer complication compare with skin traction.
