胃力康治疗胃溃疡的作用机制

Functional Mechanism of Weilikang in Treating Gastric Ulcer

目的 :测定胃溃疡 (GU)患者胃粘膜中前列腺素E2 (PGE2 )、氨基己糖含量 ,探讨胃力康治疗GU的作用机制。方法 :12 6例经胃镜证实的活动性GU伴幽门螺杆菌 (Hp)阳性的患者随机分为两组 :对照组 6 2例 ,给予奥美拉唑 2 0mg ,每晨 1次口服 ,共 4周 ,同时服用阿莫西林 1g,每日 2次 ,甲硝唑 0 .4g ,每日 2次 ,共 1周 ;治疗组 6 4例 ,除服用上述 3种药物外 ,加服胃力康 10g ,每日 3次 ,4周为 1个疗程。两组患者治疗前后测定胃粘膜组织PGE2 、氨基己糖含量。结果 :治疗组GU治愈率 92 .19%明显优于对照组 79.0 3% (P <0 .0 5 )。治疗组治疗后PGE2 含量 [(2 .82± 0 .5 8)mg/g组织 ]明显高于治疗前 [(1.4 2± 0 .33)mg/g组织 ],而对照组则无明显变化。治疗组治疗后氨基己糖含量 [(18.6 2± 2 .82 )mg/g组织 ]明显高于治疗前 [(14 .34± 2 .5 1)mg/g组织 ](P <0 .0 1) ,而对照组则无明显变化。结论 :胃力康可提高胃粘膜中PGE2 、氨基己糖含量 ,减轻胃粘膜损伤 ,促进GU愈合。

Objective: To probe into the functional me chanism of Weilikang in treating gastric ulcer by determining PGE 2 and hexosam ine levels of gastric mucosa. Methods:One hundred and twenty-six patients with active gastric ulcer and positive Hp were randomiz ed into control group and treatment group. The former (sixty-two patients) was administered with omeprazole 20 mg in the morning for 4 wks, moxicillin 1.0 g tw ice a day and metronidazole 400 mg twice a day for 1 wk. The latter (sixty-four patients) was given Weilikang 10 g orally three times a day for 4 wks besides t he above mentioned 3 drugs. Results:The cured rate of the treatment group (92.19 %) was higher than that of the control group (79. 03 %),P<0.05. In the treatment group, PGE 2 level in the gastric mucosa [ (2.82±0.58) mg/g tissue]increased significantly compared with that before th e treatment[(1.42±0.33) mg/g tissue], while in the control group, there was n o significant difference. In the treatment group, hexosamine level in the gastri c mucosa [(18.62± 2.82) mg/g tissue]increased significantly compared with tha t before the treatment [(14.34±2.51) mg/g tissue], and there was no significa nt difference between those before and after the administration in the control g roup. Conclusion: Weilikang can increase PGE 2 an d hexosamine levels of gastric mucosa, prevent the injury of gastric mucosa and promote the healing of ulcer.
