摘要
目的 :检测细胞粘附分子 (CD4 4V6)在胃癌及癌前病变的表达 ,探索其与胃癌的发生、转移及预后的相关性。方法 :用免疫组化方法检测 30例浅表性胃炎 ,34例中重度萎缩性胃炎、34例不典型增生胃粘膜及 6 4例胃癌组织中的CD4 4V6的表达。结果 :CD4 4V6的阳性表达率在浅表性胃炎、中重度萎缩性胃炎、不典型增生胃粘膜及胃癌组织分别为 3 33% (1/ 30 )、14 71% (5 / 34)、4 4 12 % (15 / 34)及 6 0 94 % (39/ 6 4 )。不典型增生及胃癌组阳性率明显高于浅表性胃炎组及中重度萎缩性胃炎组 (均P <0 .0 1) ,中重度萎缩性胃炎与浅表性胃炎组比较差异无统计学意义 ;CD4 4V6的表达与胃癌的远处转移有关 (P <0 .0 5 ) ,与TNM分期有关 (P <0 .0 1) ,与淋巴结转移无关 ,与性别、肿瘤大小、大体类型、组织学分型、浸润深度间差异均无统计学意义。结论 :用免疫组织化学方法检测胃活检标本中的CD4 4V6可能有助于胃癌早期诊断和远处转移趋势推测。
Objective:To study the expression of CD4 4 splice variant 6(CD44V 6)in gastric cancer and precancerous lesion, and discu ss the correlations of the expression of CD44V 6 and the clinical and pathologi c characteristics of gastric cancer.Methods:The ex pression of CD44V 6 was analyzed in the biopsy specimens of superficial gastrit is (30 cases),moderate-severe atrophic gastritis (34 cases),dys plastic gastric mucosa (34 cases) and gastric carcinoma tissues (64 cases) by immunohistochemistry(IHC) method .Results:The positive rate of CD44V 6 in superficial gastritis, moderat-severe atrophic gastritis, dysplastic gastric mu cosa and gastric cancer tissues was 3.33 % (1/30),14.71% (5/34),44.12 % (15/3 4) and 60.94 % (39/64) respectively. The positive rate was higher in dysplastic gastric mucosa and gastric cancer tissue than that in superficial gastritis and moderat-severe atrophic gastritis mucosa (P<0.01). The expression of CD44V 6 was related to distant metastasis (P<0.05) and TNM stage (P<0.01) but not related to lymphatic metastasis(P>05 ). No significant relations were found between the expression of CD44V 6 and sex, tumor size, site, Borrma nn type, histological type, differentiation or depth of invasion. Conclusion:Determination of CD44V 6 in biopsy specimens by IHC m ight be useful in early diagnosis of gastric cancer and predicting the potential distant metastasis.
