微生态制剂防治小儿抗生素相关性腹泻的研究 被引量:9

A study of the effect of the microeogical pregaration in childhood antibiotic correlated diarrhea

摘要 目的 观察应用三代头孢时发生抗生素相关性腹泻 (以下简称腹泻 )的因素及口服微生态制剂对其防治效果。方法 对照组 119例应用头孢曲松治疗时未同时用微生态制剂。观察组 2 19例在头孢曲松用药同时随机分组预防性给予微生态制剂 (乳酶生、双歧杆菌或双歧杆菌和酪酸梭菌二联活菌 )。结果 对照组中同年龄组头孢曲松用药剂量大于 6 0~ 80mg·kg-1 d-1易腹泻 (P =0 0 1) ;相同头孢曲松用药剂量下 ,小于 7岁者易腹泻 (P =0 0 19)。观察组腹泻的发生明显减少 (P =0 0 0 0 )。二联活菌效果较好 ,单用双歧杆菌活菌次之 (P1=0 0 2 1、P2 =0 0 14 )。腹泻发生后单纯应用微生态制剂难以见效。结论 头孢曲松用药剂量大、患儿年龄小易出现腹泻。发生腹泻后不停抗生素 ,用微生态制剂难以奏效。 Objective To observe the effect of the microeogical pregaration(probiotic) in antibiotic correlated diarrhea.Methods 338 cases witch treated by ceftriaxone were divided into two groups randomly:probiotic had or had not been applied.The probiotic groups were:(A)bioferminum,(B)bifidobactirum,(C)bifidobactirum and clostridium butyricum compositus.Results The rate of diarrhea was higher in dose of ceftriaxon 60~80mg·kg -1 ·d -1 than 30~50mg·kg -1 ·d -1 (P=0.019),and was signifycantly higher in the group of <7 years old than >7(P=0.01).When probiotic was used early,the diarrhea had an obvious reduce(P=0.000).It was showed the (C) had the best preventive effect,sole the (B) was second,and then was the (C)(P=0.021,P=0.014).Conclusion To control the ceftriaxone dosage and use probiotic compositus can prevent the antibiotic correlated diarrhea.
