摘要 目的 探讨直接数字化X线摄影 (DDR)及影像后处理在骨折影像上的应用及诊断价值 .方法 分析 4 5例外伤 ,放射科首诊可疑骨折的患部DDR图像 .由 3组医生 ,分别为DR图像不作处理组、一般处理组和增强处理组 ,对 3组诊断与临床治疗结果及随访结果对比分析 ,作出ROC曲线 .结果  4 5例中 ,有骨折 30例 ,无骨折 15例 .增强处理组敏感度最高 ,但特异性较低 ;一般处理组具有较高的敏感度、特异性及准确性 .结论 ①DR图像作适当的后处理可以提高骨折影像诊断的准确性 .②DR图像增强处理能提高骨折线细节观察的敏感度 ,但同时可能会使细节的特异性降低 ,故不应常规使用 .③加强对DR增强影像的适应性和认知力 。 Objective To investigate the use of the direct digital X-radiology and the after-finishing screenage in the fractured diagnosis. Materials and Methods We reviewed 45 patiens who were trauma and had been suspected fracture by clinical physicians and taken the direct digital X-radiology for the trauma spots. Then we chose 6 attending physicians of Radiology department and divided them into 3 groups in a randomed and claim each groups of them observing the 3 sorts of screenages of the 45 patiens in the same high resolution window of computer. The screenages involved 1.no disposal, 2.common disposal and 3.reinforcement. And then we compared the diadynamic criteria of the 3 groups with the clinical therapy results and made ROC statistical research and drew ROC curves. Results There were 30 ones being fracture and 15 ones being not fracture in the 45 patients. The screenages of reinforcement had the much highest sensitivity, but the lower specificity; the ones of the common disposal had an suitable sensitivity, specificity and accurate. Conclusions ①We can rise the accurate on the screenages diagnose through making suitable after-finishing. ②The reinforcement can significantly rise the sensitivity in observing the details but the specificity is lower,so the reinforcement should not be process for routine.③We should adapt and study the screenage of reinforcement , then we can rise the specificity on them.
