颈段气管肿瘤的外科治疗

Surgical management of cervical tracheal carcinoma

目的 探讨颈部长段气管肿瘤行人工气管替代的的外科手术治疗经验。方法 我院于 1992年 3月～ 2 0 0 1年 5月共对14例颈段气管肿瘤的患者进行了自制的人工气管替代手术治疗 ,其中原发性气管肿瘤 8例 ,继发性气管肿瘤 6例。手术方式行全喉切除、气管切除、部分食管切除、人工气管远端造瘘 11例 ;行全喉切除、气管切除、部分食管切除、人工气管两端与受体气管吻合术 3例。结果 手术切除率 10 0 %。全组无手术死亡 ,术后并发症包括吻合口肉芽 2例 ,颈部切口感染 1例。所有病例均随访 2± 3.5年 ,患者均存活良好。结论 使用人工气管替代颈段气管肿瘤切除后的长段缺损 。

Objective To explore the experience of surgical treatment of artificial tracheal substitution of the long segment cervical tracheal carcinoma.Methods From March 1992 to May 2001,14 patients with the cervical tracheal carcinoma underwent artificial tracheal substitution operation,including primary tracheal carcinoma in 8 cases,secondary tracheal carcinoma in 6 cases:11 patients underwent laryngectomy,resection of trachea,partial esophagectomy and distal artificial tracheal ostomy,3 patients underwent laryngectomy,resection of trachea,partial esophagectomy.and between artificial and original trachea.Results The resectability of cervical tracheal carcinoma was 100%, without mortality in our study.Postoperative complications involved granulation of stoma in 2 cases,infection of incisional wound of cervical in 1 case.The overall follow-up time was 2±3.5 years,and all patients livina and well.Conclusion Surgical resection of cervical tracheal carcinoma with artificial tracheal substitution operation can increase the resectability of cervical tracheal carcinoma and improve the survival quality of patients.
