经乳突乙状窦前入路的临床应用

Clinical Application of Transmastoid Presigmoid sinus Approach

目的 :探讨经乳突乙状窦前入路切除岩斜区肿瘤的可行性及优缺点。方法 :通过该入路显微手术顺利切除 8例上中下斜坡以及桥小脑角颈静脉孔区肿瘤。结果 :肿瘤完全切除 6例 ,大部切除 2例。术后并发面瘫 1例 ,脑脊液耳漏 2例。后者经外耳道填塞分别于术后 1周和 3周治愈。结论 :经乳突乙状窦前入路适用于上中斜坡。

Objective:To discuss the operative technique,advantages and postoperative complications of transmastoid presigmoid sinus approach.Methods:Eight patients with petroclival tumor treated with transmastoid presigmoid sinus approach were retrospectively studied.Results:Total resection was performed in 6 patients,and greater partial resection in 2.Major postoperative complications included Ⅶth cranial nerve palsy(1 case) and cerebrospinal fluid leakage(2 cases).Conclusion:This approach can sufficiently can the petroclival region.Postoperative complications may be related to operative technques and tumorous characteristics.
