摘要
目的 观察小儿皮肤血管瘤增生消退过程中细胞增生和凋亡状态 ,以探讨小儿血管瘤发生发展机理。方法 采用免疫组化S P法和TUNEL检测 5 8例不同阶段血管瘤组织中PCNA增殖指数 (PI)和凋亡指数 (AI)。结果 增生期血管瘤PI明显高于消退期 ,AI显著低于消退期 (P <0 .0 5 )。消退Ⅰ、Ⅱ、Ⅲ期PI逐渐降低 ,AI逐渐增高 ,除Ⅱ、Ⅲ期AI之间无显著性差异外 ,其余各组之间均有显著性差异 (P <0 .0 5 )。AI/PI在消退期明显高于增生期 ,在消退Ⅰ、Ⅱ、Ⅲ期随着病变发展明显增高 (P <0 .0 1)。结论 小儿皮肤血管瘤发生发展与细胞增生、凋亡有关。凋亡指数增高可作为血管瘤自然消退的早期判断指标。
Objective To detect the cell proliferative and apoptotic status during course from proliferation to involution in infantile hemangioma of skin,and to explore the mechanism of formation and development of infantile hemangioma.Methods The PCNA index(PI) and apoptosis index(AI) were detected with immunohistochemistry SP method and TUNEL in 58 patients with infantile hemangioma in different stage.Results The PI in protiferative hemangioma was significantly higher while the AI was significantly lower than that in involuting hemangioma (P<0.05). the PI in involuting phase Ⅰ,Ⅱand Ⅲ gradually decreased. On the other hand, the AI gradually increased.There were significant difference among different groups except for that between the phase Ⅱand Ⅲ(P<0.05).The AI/PI in involuting hemangioma was obviously higher than that in protiferative hemangioma, and increased obviously in involuting phase Ⅰ,Ⅱand Ⅲ with the progressing of the disease.Conclusion The formation and development of infantile hemangioma were related to proliferation and apoptosis of celly.Increasing of AI may be early judgement index of the natural regeression in infantile hemangioma.
