双水平气道正压呼吸机有创通气在急性呼吸衰竭患者抢救中的应用 被引量:1

摘要 目的 探讨双水平气道正压 (BiPAP)呼吸机的有创通气方式在急性呼吸衰竭患者抢救中的临床应用价值。方法 对 2 7例非肺源性的急性呼吸衰竭患者 (本身无胸廓及肺部疾患 )在采用S/T—D30型BiPAP呼吸机的有创通气方式治疗前后的动脉血气和血流动力学的变化进行比较观察。结果 治疗后 1、12、2 4h后 ,其动脉血气各项主要指标 :pH、PaO2 、PCO2 较治疗前都有明显改善 (P <0 0 5或P <0 0 1) ,且满足生理需求 ;治疗后 12、2 4h的血流动力学指标SBP、DBP、HR也较治疗前有明显改善 (P <0 0 5或P <0 0 1)。结论 采用BiPAP呼吸机有创通气方式通气可以改善肺的氧合功能 ,有助于血流动力学的稳定。因此这种通气方式完全可以满足一些急性呼衰患者的呼吸支持需要。 Objective To investigate the clinical effect of invasive bilevel positive airway pressure(BiPAP) support ventilation in patients with acute respiratory failure.Methods The change of arterial blood gas analysis and hemodynamics were analyzed before and after invasive BiPAP support ventilation in patients(who had non-chest disease and non-pulmonary diease themselves) with acute respiratory failure.Results pH, PaO 2, PCO 2 were obviously ameliorated(P<0.01or P<0.05) at 1,12,24 h after the invasive ventilation compared with those before the invasive ventilation; SBP、DBP and HR were more steady(P<0.05,P<0.01) at 12, 24 h after the invasive ventilation than those before the invasive ventilation.Conclusion The appliance of invasive BiPAP support ventilation in patients with acute respiratory failure can improve pulmonary oxygenation,and be useful for stablizing hemodynamics. Therefore, the invasive ventilation is able to provide respiration support for some patients with acute respiratory failure.
