手部烧伤后瘢痕挛缩畸形的整形治疗

Plastic Operation of Scar Contracture Malformation after Hand Burns

目的 探讨手部烧伤后瘢痕挛缩畸形的整形治疗方法。方法 1 998～ 2 0 0 3年 ,收治手部烧伤后瘢痕挛缩畸形 63例 ,行瘢痕切除矫形后固定 ,皮片及皮瓣移植修复创面。结果 48例随访 1～ 3年 ,所有受累关节得到完全或大部矫正 ,功能恢复满意。结论 整形手术应在瘢痕稳定后及早进行。创面无肌腱、骨骼裸露者宜用全厚皮片修复。需皮瓣修复者 ,手、足部皮瓣修复效果最佳。

Objective To study the plastic method of scar contracture malformation therapy after hand burns. Methods 63 cases with scar contracture malformation after hand burns were treated in 1998～2003. The scar excision and orthopedics were performed, and the traumatic surface was covered with a skin patch or flap graft. Results In 63 cases, 48 cases were followed up for 1～3 years, their malformed arthrosis was completely or mostly remedied and the function was well recovered. Conclusion Plastic operation of scar contracture malformation of hand burns should be early performed after the scar tranquilization. Using whole thick-skinned repair is held true if having no texture,and bone were emerged. The flap of hand and foot may be most effective to those patients that need repair.
