膀胱移行细胞癌CD44和nm23蛋白表达及临床意义

摘要 目的 探讨CD44和nm2 3蛋白在膀胱移行细胞癌中的表达及其与临床病理的联系。方法 应用免疫组织化学技术检测 5 5例膀胱移行细胞癌CD44和nm2 3蛋白的表达。结果  5 5例膀胱移行细胞癌中 ,CD44蛋白表达阳性率5 6 4%,且与病理分级和预后无显著性差异 (P >0 .0 5 ) ,与血管、淋巴管侵袭有显著性差异 (P <0 .0 5 )。nm2 3蛋白表达阳性率 63 6%,且随病理分级增高而降低 ,与分化程度、血管淋巴管侵袭及其预后有显著性差异 (P <0 .0 5 )。CD44和nm2 3在膀胱移行细胞癌中表达有显著性差异 (P <0 .0 5 )。CD44阳性表达伴nm2 3阴性表达者易发生转移。结论 CD44和nm2 3蛋白联合检测可以作为预测膀胱癌转移的参考指标 ,且nm2 Objective To investigate the expressions of CD44 and nm23 proteins and their clinical significance in transitional cell carcinoma (TCC) of the bladder. Methods The expression levels of CD44 and nm23 proteins in 55 cases of TCC of the bladder was detected by immunohistochemical method. Results The positive rate of expression of CD44 protein in TCC was 56.4%. There was no significant difference of the expression of CD44 with the pathological grades of tumor and the prognosis ( P >0.05), but was significantly different from the vascular and lymphatic invasion of tumor ( P <0.05). The positive rate of expression of nm23 protein in TCC was 63.6%. There was significant difference between the pathological grades Ⅰ, Ⅱ, and Ⅲ ( P <0.05). The positive rate of nm23 in TCC was significantly different from tumor differentiation and vascular and lymphatic invasion of tumor. CD44 expression was significantly different from nm23 expression in TCC ( P <0.05). The positive expression of CD44 and negative expression of nm23 were closely associated with the TCC metastasis. Conclusion The expression of CD44 and nm23 proteins may be a predicting factor of metastasis of TCC. The detection of nm23 protein might be useful to predict the prognosis of bladder cancer.
