十二指肠降段出血疾病的胃镜特点和临床分析

Endoscopic Features and Clinical Analysis of Bleeding resulted from Duodenal Decending Part Diseases

目的 探讨十二指肠降段病变并出血的病因、主要临床表现及胃镜下改变。方法 总结本院 10年来胃镜诊断十二指肠降段病变并出血 4 0例 ,分析其临床表现及胃镜下改变。结果 十二指肠降段病变并出血病因复杂 ,以降部溃疡最常见 ,占 4 2 .5 % ,其次为降部息肉 2 5 % ,降部癌肿 15 % ,降段钩虫感染 5 % ,降段憩室炎 5 % ,降段炎5 % ,降段血管病变 2 .5 %。其出血以黑便为主 10 0 % ,呕血占 2 0 %。降部溃疡出血以单发溃疡多见 ,息肉以单发尤其表面糜烂者多见 ,癌肿以进展期常见。结论 十二指肠降段病变并出血病因复杂 ,临床表现缺乏特异性 ,胃镜对其诊断有重要价值。

Objective To probe into the cause , clinical manifestation and gastroscopy of bleeding of duodenal descending part disease.Methods Gastroscopic features of 40 cases of bleeding of duodenal descending part disease in recent 10 years in our hospital were reviewed.Results The data presented that the cause of bleeding of duodenal descending part disease was complicated:descending part ulcer(42.5%),descending part tumor(25%),descending part cancer(15%),descending part hookworm infection(5%),inflammation of descending part diverticulum (5%),inflammation of descending part(5%),descending part blood vessel disease(2.5%).The bleeding with black shit(100%),haematemesis(20%).moreover, single ulcer and single tumor and superficial ulcerate and cancerous swelling during the worsening period are easy bleeding.Conclusions The gastroscope diagnosis has great value to the bleeding of duodenal descending part disease, and the cause of duodenal descending part disease is very complicated and its clinical manifestation is not typical.
