漂浮导管在预防高龄患者前列腺电切综合症中的应用

目的探讨高龄患者应用漂浮导管使用甘露醇冲洗液在前列腺电切中对循环系统的影响。方法 30例高龄患者随机均分为甘露醇组和葡萄糖组,采用硬膜外麻醉,经右颈内静脉插入漂浮导管,记录心排量,肺毛细血管契压等。结果 两组血液动力学变化方向相同,但甘露醇组更易发生高血容量。结论 无论采用甘露醇或葡萄糖做冲洗液,都不能避免液体吸收。对高龄患者必须严密监护血容量变化。

<abstract>jective To determine the hemodynamic changes during TURP. Methods 30 patients aged from 70~88 were randomly divided into group A (irrigated with 5%mannitol) and group B (irrigated with 5% glucose). Epidural anesthesia were employed, Swan-Ganz catheter was inserted from the right internal jugular vein,vital signs and hemodynamic data were collected every 15 min. Results CO and PWP were decreased gradually till 30 min then increased , at 60min CO and PWP in group A were significantly higher than that in group B. Conclusion TURP irrigated with mannitol may cause relatively expanded blood volume than that with glucose, and sometimes need more intervention.
