肝动脉化疗栓塞术加PEI介入治疗原发性肝癌的方法和临床研究

目的本研究课题是在超选择性肝动脉化疗栓塞术(TOCE)的基础上,在CT的引导下,加用经皮穿刺瘤内无水乙醇注射术(PET)来治疗原发性肝癌,期望提高治疗肝癌的疗效和减少并发症的发生.方法统计96例病人,男75人,女14例,年龄30～70岁,平均53岁.经CT、AFP、和肝动脉造影诊断为肝癌,行肝动脉化疗栓塞术+PET术.对照组25例为行单纯的肝动脉化疗栓塞术.结果行肝动脉化疗栓塞术+经皮穿刺瘤内无水乙醇注射术的96例病例中,随访86例,其中有25例死亡,存活＜6个月者20例(23%);＞6个月者11例(13%);12个月以上者56例(67%).而对照组20例行单纯的肝动脉化疗栓塞术的病例中,12月以上的存活6例(30%).结论肝动脉化疗栓塞术+经皮穿刺瘤内无水乙醇注射术的治疗方法过程安全性高、经济、简便易行、还可从某种程度起到双重栓塞的作用,疗效显著,对肝功能影响亦较少.

<abstract>jective With the base of TOCE, curing Primary Hepatic Carcinoma by PET,to improve the treatmeng of Primary Hepatic Carcinoma and decrease the side effect. Methods With 96 cases , including 75 male patients and 14 female patients , the age is 30~70 and the averge is 53, which have been diagnosted Primary Hepatic Carcinoma by CT, AFP and hepatic Arterial Urography to TOCE and PET. The contrast only use the hepatic arterial chemotheraphy embolization. Result The 96 cases with TOCE and PET ,25 cases death , the lower than six months is 20 cases(25%), exceed six months was 11 cases(13%), exceed 12 months was 56 case(67%). But only 6 cases which exceed 12 months in the 20 cases with only using hepatic arterial chemotheraphy embolization. Conclusion The TOCE and PET can play dual emboliztion role and hand no servious damage to liver function.
