摘要
目的 观察氟伐他汀 2 0mg/d对原发性高胆固醇血症患者血脂的影响及对血管内皮功能保护作用。 方法 6 7例初诊未曾应用药物治疗的原发性高胆固醇血症患者每晚服用氟伐他汀 2 0mg/d ,于用药前及用药 4周检测血脂、血液生化 ,并应用放免法测定血浆内皮素 (endothelins,ET)及酶法测定一氧化氮 (nitrogenmonoxide,NO)水平。结果 用药 4周后 ,血清总胆固醇 (totalcholestero ,TC)水平下降 1 8.1 %。低密度脂蛋白胆固醇 (lowdensitylipoproteincholestero,LDL C)下降 2 4 .9%,高密度脂蛋白胆固醇 (highdensitylipoproteincholestero ,HDL C)升高7.5 %,血清甘油三酯 (triglycerides,TG)下降 1 1 .1 %。这四项指标均有统计学意义 (P =0 .0 38,P =0 .0 0 1 ,P =0 .0 2 5 ,P =0 .0 1 )。用药 4周后 ,ET有所下降 ,NO及NO/ET明显升高 (P =0 .0 96 ,P =0 .0 4 2 ,P =0 .0 2 5 )。治疗前后血清生化指标无显著变化。结论 氟伐他汀有效治疗高脂血症的同时 ,具有良好的内皮保护作用。
Objective To investigate the efficacy and safety of Fluvastatin in improving endothelial function in patients with hyperlipidemia.Methods Fluvastatin 20 mg/d was used in 67 patients with hypercholesterolemia.Before and after 4 weeks' treatment,serum lipids,lipoproteins,other biochemistry data,plasma endothelin (ET) and nitrogen monoxide(NO) were examined.Results After 4 weeks' treatment,serum level of total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL C) significantly decreased 18.1%?11.1%?24.9% respectively( P =0.038, P = 0.001 , P =0.025, P =0.01),and high density lipoprotein cholesterol (HDL C) increased 7.5% significantly ( P = 0.01 ).NO and the ratio of NO/ET increased significantly ( P =0.096, P =0.042, P =0.025).Other indices in biochemistry did not change significantly.Conclusion Fluvastatin with the dose of 20 mg/d is not only effective and safe,but also improves vascular endothelial function in treating patients with hyperlipidemia.
