摘要
目的 探讨多重受体阻断对不同血管紧张素Ⅱ (AngⅡ )水平高血压患者血管活性物质 ,氧化 /抗氧化物及血压的影响。方法 筛选轻中度高血压患者 5 6例 ,其中AngⅡ≤ 5 0ng/L和>5 0ng/L者各 2 8例 ,前者分为A、B两组 ,后者分为C、D两组 (各组 14例 )。A、C两组服用缬沙坦( 160mg/d) ,B、C两组服用缬沙坦 ( 160mg/d) +卡维地络 ( 2 0mg/d) ,共治疗 8周 ,治疗前后测定上述研究指标。结果 与治疗前比较 ,治疗后各组血压、血浆氧化产物 (MDA)及D组AngⅡ和内皮素 (ET)均有明显下降 (P均 <0 .0 5 ) ,而各组抗氧化物 (SOD)和D组一氧化氮 (NO)在治疗后有明显上升 ,但B、D两组SOD和MDA的上升或下降幅度均大于A、C两组 (P均 <0 .0 5 ) ,而D组的变化更为显著 (P均 <0 .0 1)。结论 阻断AngⅡ 1型受体 (All IR )对AngⅡ大于 5 0ng/L的高血压患者有明显的内皮功能改善作用和抗氧化效应 ,在阻断AⅡ IR的同时阻断 β 受体和a 受体 。
Objective To explore the effects of blocking multi-receptor on blood pressure(SBp/DBp),AngiotensinⅡ(AngⅡ),Endothelin-I(ET),nitric oxide (NO),Superoxide dismutase (SOD) and Malondialdehyde(MDA) in serum for essential hypertension patients(EH) accompanied by different Serum AngⅡ.Methods choosing mild to moderate EH 56 cases,28 patients with AngⅡ≤50ng/L were divided into Group A and Group B.Other 28 cases with AngⅡ>50ng/L were divided into Group C and Group D (n=14 ).Group A and C were administrated by Valsartan(Val) 160mg/d,Group B and D were administrated by val 160mg/d + carvedilol(Car) 20mg/d for 8 weeks.Before/after of therapy,the SBp/DBp,ET,AngⅡ,NO,SOD and MDA in serum were measured or assessed.Results The SBp/DBp MDA each group and serum AngⅡ and ET in group D were significantly decreased,But serum SOD each group and serum No in group D were significantly increased compared with before therapy,the changes in group B and D were more significantly.Conclusions Multi-receptor blocking can significantly improve the endothelial function and antioxidation than single receptor blocking,for the EH patients accompanied by high level AngⅡ. \[
