Expression of Cyclooxygenase-2 and Inducible Nitric Oxide Synthase and Their Significance in Endometrial Carcinoma

摘要 目的 探讨环氧合酶 2 (COX 2 )与诱导型一氧化氮合酶 (iNOS)在子宫内膜癌组织中的表达及与子宫内膜癌发生和进展的关系。方法 应用免疫组化方法检测 30例子宫内膜癌组织中COX 2和i NOS的表达 ;并以逆转录聚合酶链反应 (RT -PCR)技术检测COX 2及iNOSmRNA的表达。结果 子宫内膜癌组织中COX 2及iNOS蛋白阳性表达率分别为 6 6 .7%和 73.3% ;子宫内膜癌及癌旁组织中COX 2及iNOSmRNA的表达均明显上调。中分化内膜癌组织COX 2蛋白的表达显著高于低分化的癌组织 (P <0 .0 5 )。子宫肌层浸润深度 >1/2的iNOS表达明显高于肌层未受累或≤ 1/2的患者 30例 (P<0 .0 5 )。COX 2和iNOS蛋白的表达呈明显的正相关 (r =0 .6 0 1,P <0 .0 0 1)。结论 COX 2和iNOS的高表达与子宫内膜癌的发生及发展密切相关 ;COX 2可能主要参与子宫内膜癌发生的早期 ; Objective To investigate the expression of COX-2 and iNOS in endometrial carcinomas and to evaluate the correlation between the expression of COX-2,iNOS and clinicopathologic variables in these patients.Methods The expression of COX-2 protein and iNOS protein in thirty endometrial carcinomas surgical specimens were examined by immunohistochemical technique. COX-2 and iNOS mRNA expression was examined by reverse-transcription polymerase chain reaction (RT-PCR) in endometrial carcinomas,adjacent nontumourous tissues, and normal endometrium. Results Upregulation of COX-2 and iNOS mRNA expression were confirmed in endometrial carcinomas and adjacent nontumourous tissues compared to normal endometrium. COX-2 and iNOS expression were increased in 20 of 30 ((66.7%)) and 22 of 30 ((73.3%)) cases, respectively. The moderated differentiated endometrial carcinomas expressed COX-2 protein stronger than poor differentiated ovarian carcinomas (P<(0.05)). Although iNOS did not correlate with any conventional clinicopathologic parameters in these patients. iNOS-positive rate was significantly higher in those cases that myometrium invasion with invasion exceeding 1/2 than those without invasion or with invasion not exceeding 1/2; COX-2 expression was significantly correlated with iNOS expression (r=(0.601),P<(0.05)).Conclusion The combine expression of COX-2 and iNOS may play an important role in the development of endometrial carcinomas. COX-2 may involve early in endometrium neoplastic progression. Excessive expression of iNOS may contribute to invasion of endometrial carcinomas.
