摘要 目的 探讨骨肉瘤中CD4 4v6的表达与其临床及生物学行为的关系。方法 应用流式细胞免疫学方法检测 4 0例骨肉瘤、10例骨巨细胞瘤、10例骨软骨瘤石蜡标本的CD4 4v6表达情况 ,综合分析检测结果对骨肉瘤侵袭转移及预后的影响。结果 CD4 4v6在骨肉瘤中高表达 ,明显高于骨巨细胞瘤及骨软骨瘤。CD4 4v6表达与骨肉瘤患者的性别、年龄、病程及肿瘤发生部位无关 ;在Enneking外科分期≥ⅡB期 (ⅡB、Ⅲ期 )、局部复发、伴有肺转移的骨肉瘤中 ,其CD4 4v6表达值明显高于 <ⅡB期 (ⅡA期 )、局部未复发、无肺转移的骨肉瘤。结论 CD4 4v6可作为骨肿瘤恶性表型的标志和诊断的辅助指标。CD4 4v6阳性可能预示骨肉瘤复发或转移的倾向 。 Objective To explore the expression of CD44v6 protein in osteosarcoma and study the relationship between the expression of CD44v6 protein and clinical behavior of osteosarcoma. Methods The expression of CD44v6 protein was detected by Flow Cytometry in the paraffin-embedded tissues from 40 cases of osteosarcoma, 10 cases of giant cell tumor of bone, and 10 cases of osteochondroma tissues were examined as control. The expression of CD44v6 in patients with osteosarcoma and their effects on tumor recurrence, metastasis and prognostic significance were studied. Results The positive expression rate of CD44v6 in osteosarcoma was significantly higher than that in giant cell tumor of bone and osteochondroma. There was no significant correlation between the CD44v6 positive rates and clinicopathological features such as age, sex, locations of lesion and prognosis. The high level expression of CD44v6 in osteosarcoma tissues was associated with its Enneking staging. CD44v6 positive rates in staging≥ⅡB were much higher than that in staging<ⅡB(P<0.05). Osteosarcoma recurrence showed significantly higher positive rates than non-recurrence(P<0.05). CD44v6 in metastasis were significantly higher than that in non-metastasis(P<0.05). Conclusion CD44v6 may act as an indicator in the grade of malignancy of osteosarcoma. It is also an adjuvant index in the diagnosis of osteosarcoma. Overexpression of CD44v6 might be an indication for predicting the possibility of recurrence metastasis of osteosarcoma.
