摘要
目的了解癫儿童应用抗癫药 (AEDs)治疗后血清胰岛素水平的变化情况。方法按选用不同的AEDs(单药治疗 ) ,将 66例 46岁的癫患儿分成丙戊酸钠治疗组 (治疗组 )和非丙戊酸钠治疗组 (对照组 ) ,从服用AEDs开始至治疗后6个月 ,测定其餐前胰岛素水平。结果治疗组中 ,治疗后 3个月与治疗后 1个月血清胰岛素水平相比 ,P <0 0 1,有显著差异 ;治疗组与对照组比较 ,治疗后 3个月与治疗后 6个月胰岛素水平相比 ,P <0 0 1,有显著差异。结论本研究结果表明 ,癫患儿应用丙戊酸钠治疗后可导致血清胰岛素水平明显升高 ,其发生时间在应用丙戊酸钠治疗后 3个月开始 ,这与应用丙戊酸钠治疗后导致体重指数 (BMI)值出现明显改变的情况相吻合 ,其发生机制可能与应用丙戊酸钠治疗后 ,患儿产生胰岛素耐受有关。
Objective To find out the alteration in serum level of insulin in epileptic children after the administration of different antiepileptic drugs (AEDs). Methods According to the type of AEDs,66 children aged 4 to 6 year with epilepsy were assigned randomly into two groups,Valproate sodium group,the study group,and none-Valproate group,the control group.At the beginning and during the 6-month period of treatment with AEDs,pre-prandial serum level of insulin was determined at various time points and compared between the two groups. Results In the study group,serum insulin level was significantly different after the first month from that after the third month of the treatment P<0.01;between the study and control groups,at the ends of the 3rd and 6th month,the serum levels of insulin were significantly different from each other P<0.01. Conclusions The present study showed that the level of serum insulin would be increased obviously after administration of Valproate sodium in epileptic children.The elevation usually began 3 months after the treatment.This was in accordance with the alteration in Body Mass Index (BMI) induced by Valproate sodium.The mechanism for change in serum level insulin might be insulin resistance resulted from administration of Valproate sodium.
