内镜下局部注射三种药物治疗消化性溃疡出血的疗效对比

COMPARISON OF THREE KINDS OF MEDICINE TO TREAT THE PEPTIC ULCER CAUSED HEMORRHAGE WITH LOCAL INJECTION UNDER ENDOSCOPES

目的 分别采用立止血、1 /1 0 0 0 0肾上腺素和生理盐水对 89例消化性溃疡 (PU)出血症进行内镜下局部注射治疗 ,对比三种药物的止血疗效。方法 检查治疗时机均在 1 2～ 2 4小时之内 ,明确病灶性质后 ,距病灶 1mm～ 3mm周围 ,分 4～ 6个点局部注射以上任何一种药物 ,每点注射 0 .2ml～ 1 .0ml,直至内镜下观察到病灶出血停止。结果 三组 48小时止血率、1周内再出血率和外科手术率 ,经统计学检验无明显差异 (P >0 .0 5 )。结论 立止血、肾上腺素和生理盐水在用于内镜注射治疗PU出血中的疗效令人满意 ,其中任何一种药物均可作为内镜注射治疗PU出血的选择用药。

Objective To compare the hemostasis effect of three kind drugs, namely Reptilase, Adrenaline (1/10000) and Normal Saline, on treatment of 89 cases of patients with peptic ulcer (PU) caused hemorrhage with local injection under endoscopes. Methods The chance of treatment was chosen within 12～24 hrs. After defined the property of the focus, any of the drugs mentioned above was locally injected around 1 mm～3mm of the focus. The injection points were 4～6. Each point injected 0.2ml～1.0ml of drug until the bleeding was stopped when observing under endoscopes. Results All of three groups concerning the rate of hemostasis in 48 hrs; ulcer-bleeding recurrence within 1 week and the rate of performing operation have not shown significant difference (P<0.05). Conclusion Reptilase, Adrenaline (1/10000) and Normal Saline showed satisfactory effect on the treatment of PU hemorrhage by means of injection via endoscopes. Any of them can be used as an effective drug for treatment of PU bleeding via endoscopes injection therapy.
