成人特发性血小板减少性紫癜血小板膜糖蛋白测定及其临床意义研究

The Detection and Clinical Significance of Platelet Membrane Glycoprotein in Adult Idiopathic Thrombocytopenic Purpura

目的 :研究成人特发性血小板减少性紫癜 (ITP)血小板膜糖蛋白阳性率与疗效的关系。方法 :利用流式细胞仪、单克隆抗体测定 73例成人特发性血小板性紫癜 (IdiopathicThrombocytopenicPurpura ,ITP)和 19例正常对照血小板膜糖蛋白GPⅡa、GPⅨ、GPⅢa、GPIb、GPⅡb阳性率。观察血小板膜糖蛋白阳性率与成人ITP疗效的关系。结果 :19例正常对照组阳性率均 >97% ;4 0例GP阳性率 <85 %ITP患者组疗效明显低于阳性率≥ 85 %ITP患者组 (F =18 2 5 ,P <0 0 0 5 ) ;5 1例女性ITP患者组疗效明显低于 2 2例男性ITP患者组 (F =8 4 2 4 ,P <0 0 5 ) ;成人ITP患者疗效与血小板膜糖蛋白阳性率呈明显正相关 (R =1 0 ) ,73例成人ITP血小板膜糖蛋白阳性率愈低治疗疗效愈差。成人ITP血小板膜糖蛋白阳性率在一定程度上反应患者病情的程度 ,为成人ITP初诊患者选择治疗方案提供可行性实验依据。

Objects: To investigate the correlationship between the platelet membrane glycoproteins(GP) positive rate and the treatment response rate in adult idiopathic thrombocytopenic purprua (ITP). Methods: Five types of platelet membrane of glycoproteins (GPⅡa,GPⅨ,GPⅢa,GPⅠb,GPⅡb) in 73 ITP patients and 19 normal controls were detected by flow cytometer through the corresponding monoantibodies. Results: The GP positive rates were all above 97% in the 19 normal controls. The GP positive rates were lower be low 85% in 40 cases of the 73 ITP patients,and higher over 85% in the other 33 cases. There was significant statistic difference between the two groups (F value 18 25,P <0 005). Response rates of 51 female patients were significantly lower than 22 males (F value 8 424,P <0 05). The GP positive rate were positively correlated with clinical response rates in adult ITP( r =1 0). Conclusion: The platelet membrane GP positive rates of adult ITP could imply the disease status in a way and provide experimental evidence for the choice of therapeutic regimens.
