儿童伤害治疗转归及影响因素研究

Research on the therapeutic prognosis and influencing factors for injuries in children

【目的】 分析 0～ 14岁儿童伤害治疗转归及影响因素。 【方法】 采用多项有序反应变量Logistic逐步回归方法 ,分析影响儿童伤害治疗转归的有关因素。 【结果】 武汉某医院 1996～ 2 0 0 0年 0～ 14岁儿童伤害占全部伤害的 16.0 % ;主要原因依次为交通事故 ( 3 1.1% )、意外跌落 ( 2 1.7% )、意外窒息 ( 14 .0 % )、机械事故 ( 11.8% )和意外中毒 ( 8.5 % ) ;从治疗转归来看 ,治愈率为 71.7% ,死亡率为 2 .4% ;影响儿童伤害治疗转归的有关因素 :入院病情、伤害部位、手术、抢救次数、抢救成功次数和伤害原因。 【结论】 正确评估院前伤害病情 ,提高医院急救水平 ,可有效改善儿童伤害治疗质量。

Objective To analyze the therapeutic prognosis and influencing factors for injuries in children aged 0～14 years. The influencing factors which affected the therapeutic prognosis for injuries were analyzed by using multi-ordinal-categories Logistic regression model. The injury proportion of children aged 0～14 years accounted for 16.0% among all the hospitalizing cases which were admitted to Wuhan hospital for injuries during 1996～2000. The main causes for injuries were traffic accident(31.1%),unintentional falls(21.7%),accidental asphyxia(14.0%),machine accident(11.8%),and unintentional poisoning(8.5%) respectively.The cure rate for injuries was 71.7% in contrast to the mortality rate(2.4%). The influencing factors of therapeutic prognosis for injuries were injury severity,wound positions,operation, rescue times, succeeding rescue times,and causes for injuries. [Conclusion] In order to effectively improve the therapeutic prognosis for injuries in children, it is needed for the hospital to evaluate pre-hospital states for injuries correctly, and to improve the hospital ability to deal with emergence.
